DOWNLOAD OUR CASE INFORMATION FORM and fill out the information requested.
Then save the form and print it.
Our Case Information Form is available
in two formats - PDF and Word (click on the one that you prefer to download and
fill out). The PDF form can be opened and filled out online, and then saved to
your local computer.

AFTER
FILLING OUT AND SAVING THE CASE INFORMATIUON FORM AND PRINTING IT, SUBMIT IT TO
WESERVEIT ALONG WITH YOUR DOCUMENTS AS FOLLOWS:
1 - Fax Case Information Form(s) and all documents referenced therein to:

- Scan all documents and create a .zip file (if sending multiple docs) and then
email the .zip file or individual docs (if non-multiple) to: info@weserveit.com
On the subject line of your email state:
Documents Attached
Do not send any single
email that exceeds 2MB, including attachments. If
a single email transmission exceeds this limit, please submit several emails. |
